The vaporscan helmet is an exotic headgear. It can only generate when playing as the Technician. It increases damage dealt to enemies in smoke.

Smoke screamer: enemies in smoke take +50% damage from your attacks

The damage increase affects enemies in either regular smoke or toxic clouds. However it works by checking if the targeted enemy is in the cloud, and then applies a 50% damage bonus to the player character. So that means if you are using something like a shotgun you need to target the enemy to get the bonus, but then the bonus applies to all enemies hit by the attack.
